This information may include, but is not limited to, medical conditions, prior procedures, medications, allergies, pregnancy or breastfeeding status, previous reactions to medications or treatments, and other health factors that may affect whether a procedure is appropriate or require additional clinical consideration.
Patients are responsible for providing complete and accurate information and for informing INJX BY NAT LLC of any changes to their medical history, medications, allergies, or health status. Treatment may be postponed, modified, or declined when additional medical information or clinical clearance is necessary.
